Job Description

Conduct nursing assessments for people with developmental disabilities

Play an essential role in the development of more effective and efficient health care coordination for our clients with developmental disabilities, including those moving out of institutional settings;

Utilize their clinical skills to assist HRC to meet clients’ health needs throughout the continuum of care, including those moving out of institutional settings;

Actively participate as a member of a multidisciplinary care team to address ongoing needs of HRC clients in multiple environments;

Provide consultation and training for social services staff who provide case management services for people with developmental disabilities and their families;

Provide guidance and support to HRC clients, family members, caregivers, physicians and other health care providers to facilitate the best options to meet clients’ health care needs including the effective management of health care benefits and costs;

Be an advocate for clients to the full extent of their existing health care coverage and programs;

Provide oversight/monitoring, consultation, technical assistance and training for community based services providing health care to people with developmental disabilities;

Conduct nursing assessments for people with developmental disabilities;

Conduct needs assessments and develop new health-related programs for children and adults with developmental disabilities


  • B.A. Degree;
  • Registered Nurse with current CA state license and a minimum of three-five years recent clinical nursing experience;
  • At least three years direct experience with persons with developmental disabilities preferred;
  • Ability to exercise independent and sound judgment in decision making, utilizing all relevant information;
  • Effective communication skills to facilitate collaboration among all members of the healthcare team is essential;
  • Good time management and organizational skills along with the ability to manage continually changing priorities;
  • The ideal candidate will have excellent written and verbal communication skills in addition to knowledge of Excel and Word
