DDS Family Outcomes Survey

The Department of Developmental Services (DDS) recently mailed and emailed surveys to families  who have participated in the Early Start Program. DDS uses the feedback received from these surveys to create a better experience for new families participating the in the Early Start Program. Please complete the survey by November 30, 2022.
