HRC has contracted with Phoenix Facilitation to provide a coaching program for Self-Determination Program (SDP) participants.   

If you have decided to move forward with the SDP and are having challenges with navigating any stage of the program and have not yet achieved “live” status, please let your service coordinator know.  You can also visit the Phoenix Facilitation Website. 

Coaching support can include: 

  • extra training & information on specific components/stages of the program,
  • extra support with identifying and securing a Person-Centered Planner (PCP), Independent Facilitator (IF), and/or Financial Management Service (FMS) agency
  • extra support with creating a Spending Plan,
  • and extra support with identifying providers to deliver a service to you.
