Dear Harbor Service Provider,

We are writing regarding the upcoming Quality Incentive Program (QIP) for fiscal year (FY) 2026-2027 and the action required by service providers in order to earn the 10% QIP portion of the rate.

As outlined in the Department of Developmental Services (DDS) Directive D-2025-Quality Incentive Program-015, beginning November 3, 2025, DDS is collecting data from service providers with QIP-eligible purchase of service (POS) authorizations and activities during FY 2024-25. You should have received an email from DDS (DDS-OQA@qualtrics-survey.com) that included a hyperlink to an online survey. This email was sent to the email address you have registered in the Provider Directory and had a reference to “Survey Invitation” in the subject line, for example, “Provider Capacity Survey Invitation.”

Data collected from eligible service providers will fall under three reporting measures:

  • Provider Capacity Measure: All service providers delivering QIP-eligible services are required to complete the Provider Capacity Measure. The directive can be found here.
  • Employment Measure: Employment service providers (service codes 950 and 952) are required to complete the Employment measure (Report on staff training and CIE placements) and Provider Capacity measure (Report on provider characteristics and workforce capacity). The directive can be found here.
  • Prevention and Wellness Measure: Residential service providers (service codes 113, 904, 905, 915, 910, and 920) are required to complete the Prevention and Wellness measure (Report on preventative health screenings for child and adult residents) and Provider Capacity measure (Report on provider characteristics and workforce capacity). Service providers must submit complete reporting by January 31, 2026, and meet all compliance requirements in the directive referenced above to qualify for their Quality Incentive rate component in FY 2026-27.

Additionally, as a reminder, existing service providers must be in compliance with all of the following requirements, as applicable to each provider, by February 27, 2026, to be eligible to earn the 10% QIP portion of their rate for FY 2026-27:

  • Electronic Visit Verification (EVV)
  • Home and Community-Based Services (HCBS) Settings Final Rule
  • Independent Audits and Reviews
